Title : ( Difficulties in the diagnosis and management of alveolar hydatid disease: A case series )

Abstract

Summary: Objectives: Alveolar echinococcosis is a chronic, rare and sometimes lethal parasitic infection in humans, caused by the larval stage of the fox tapeworm Echinococcus multilocularis.Echinococcus multilocularis behaves biologically similar to a malignant tumor characterized by destructive tissue growth and formation of metastases in distant organs with a high morbidity and mortality rate in untreated patients. The Purpose of the study is to analyze and evaluate the clinical aspects and the treatment outcomes of patients referred to us for treatment of this disease. Materials and methods: The medical records of all patients with alveolar echinococcosis diagnosed and treated in two university hospitals of Ghaem and Omid, Mashhad university of medical sciences, between 1997 and 2012 were reviewed. The data was analyzed using SPSS R statistical software (version 213.02). The qualitative variables were alnalysed using frequency and percent in describtive statistics and chi-square test as an inferential statistics. Also, we have used the mean and standard deviation to describe the numerical variables. Results: The number of 18 patients are investigated in the study. The mean age of the 18 patients was 47.346.11+15.14 years (median of 47 and range of 23-74 yeras). There was a predominance of women. (14, 77.78% females vs 4, 22.22% males) and the differnce was significant (p=0.012). 14 (77.78%) patients were from around the city of cChenaran town. (razavi kKhorasan Razavi province). Different procedures were performed including complete liver resection in 7 (38.89%) patients, biliary bypass in 2 (11.11%) patients, laparotomy and tumor biopsy in 8 (44.44%) patients and long term medical treatment in one (5.56%) patient. The distribution of the procedure was significantly different (p=0.042). The lung was involved in 4 (22.22%) patients. 4 (22.22%) cases were died with a mean survival time of 45.70+7.50 months after onset of the disease. At present 14 (77.78%) patients are alive with medianmean follow up time of 54.60+29.17 months and the difference was significant (0.012). Conclusion: Alveolar echinococcosis could be a health problem issue in some areas of our communities. There is still a need for increasing our experience aimed at preventive, diagnostic and therapeutic measures.
