Abstract

Rectal diverticulum is an out pouching of the rectal mucosa through a vent in the overlying muscle layers and is usually found in middle aged, male dogs. A 12-year-old Terrier male dog, weighing 6 kg, was admitted to the Department of Surgery, Faculty of Veterinary Medicine, Ferdowsi University of Mashhad, with a 5 months history of a left perineal swelling, dyschezia and tenesmus. The perineal swelling was hot, soft and painful. Digital Rectal examination revealed a weakness in the left pelvic diaphragm and a large out-pouched pocket situated at the left lateral aspect of the rectal wall approximately 2 cm from the anal verge was digitally palpable. Barium enema examination showed a marked solitary 5 cm diameter diverticulum with wide-orifice neck arising from the left rectal wall. After the confirmation of rectal diverticulum, removal of the diverticulum with lateral approach and repair of the hernia were performed. The laterally curved, dorso-ventral skin incision was done over the hernia, extending from the spot just lateral to the tail base to the medial angle of ischial tuberosity. A 6×6×4 cm faeces-filled mucosal pouch was protruding through a rent in the rectal muscular tunic. After resection of all bulging mucosa, the rectal wall was closed using simple interrupted suture pattern, oversewn with a continuous cushing pattern through the serosal and muscular layers parallel to rectal direction. The anal sacs were removed with close technique. After removal of the diverticulum, anatomical structures were exposed and the hernia was repaired. Routine closure of the subcutaneous tissues and skin were performed. The dog defecated normally with 1–2 bowel movements per day during the 12 months following the operation.
